About the Role
As a Disability Support Worker with Concept Care, you will provide in-home and community-based support tailored to each individual’s needs.
Your responsibilities may include:
- Assistance with daily living activities, personal care and medication prompts
- Supporting clinical care requirements such as PEG feeding, bowel care, stoma care and catheter care
- Transport assistance to appointments, shopping and social activities
- Implementing positive behaviour support strategies when required
- Encouraging independence, skill development and community participation
This is a varied role where your consistency, presence and approach make a genuine difference.
Who We’re Looking For
We’re seeking people who are dependable, respectful and calm under pressure.
You’ll be well-suited to this role if you:
- Communicate clearly and respectfully
- Remain composed in challenging situations
- Understand professional boundaries and confidentiality
- Follow processes and complete documentation accurately
- Work collaboratively with clients, families and team members
- Are open to learning, feedback and ongoing development
Experience supporting people with complex behaviours, mental health conditions, ABI, Autism or high-support needs is highly regarded. A Certificate IV in Disability, Aged Care or Community Services is preferred, but not essential for the right candidate.
A current Australian driver’s licence and access to a reliable vehicle are required.
